ABSTRACT

BACKGROUND@#There is little data on the association between the lower nutrition represented by serum albumin levels and related factors in a general population. The present study aimed to determine whether the albumin level positioned as some kind of biomarker with frailty measures, trace elements, and an inflammation marker.@*METHODS@#In 2018, we performed an epidemiological survey in 1368 subjects who resided in Tanushimaru, Japan, in which we examined the blood chemistry including albumin, trace elements, hormone levels, and carotid ultrasonography. Albumin levels were categorized into 4 groups (G1 [3.2-3.9 mg/dL], G2 [4.0-4.3 mg/dL], G3 [4.4-4.6 mg/dL], and G4 [4.7-5.3 mg/dL]). The participants underwent measurements of handgrip strength and were tested by asking to walk 5 m. Their cognitive functions were evaluated by the mini-mental state examination (MMSE).@*RESULTS@#Multiple stepwise regression analysis demonstrated that albumin levels were significantly and independently associated with age (inversely), systolic blood pressures, estimated glomerular filtration rate (eGFR), MMSE score, frailty measures (handgrip strength), an inflammation marker (high-sensitivity C-reactive protein), hormones (growth hormone (inversely) and insulin-like growth factor-1), and trace elements (calcium, magnesium, iron, and zinc), with a linear trend.@*CONCLUSIONS@#Lower albumin levels, even in the normal range, were found to be related factors of frailty measures, trace elements, and an inflammation marker in a general population.

ABSTRACT

ABSTRACT Objective: To evaluate the association of body mass index (BMI) and albumin with pulmonary function in cystic fibrosis (CF) pediatric subjects. Methods: This is a cross-sectional study with clinically stable CF's subjects. Clinical (pulmonary function) and nutritional evaluation (body mass index and albumin) were performed. Univariate analysis was performed using simple linear correlations. Regression analysis was performed using an exit level of p<0.05. Results: Seventy-eight CF's subjects (mean age 12.8±3.8 years) with mean albumin 4.2±0.4 mg/dL, predicted forced expiratory volume in 1 second (FEV1%) 80.8±22.6 and BMI median percentile 51.2 (1.3-97.7). In the multiple regression models, albumin, age and BMI percentile were associated with pulmonary function. Subjects with lower than 25 BMI percentile had 12.2% lower FEV1%. An albumin increase of 0.1 mg was associated with 2.7% increase in predicted FEV1%, and one year increase in age was associated with reduction in 1.2% of predicted FEV1%. Conclusions: BMI percentile, albumin and age were independently associated with predicted FEV1% in a tertiary referral hospital.


RESUMO Objetivo: Avaliar a associação do Índice de Massa Corporal (IMC) e da albumina com a função pulmonar em pacientes pediátricos com fibrose cística (FC). Métodos: Estudo transversal com pacientes pediátricos com FC clinicamente estáveis. Foram realizadas avaliação clínica (função pulmonar) e nutricional (IMC e albumina). Análise univariada foi realizada usando correlação linear simples. Análise de regressão foi realizada usando o nível de significância de p<0,05. Resultados: Foram incluídos 78 pacientes com FC (média de idade 12,8±3,8 anos) com média de albumina de 4,2±0,4 mg/dL, volume expiratório forçado em um segundo (VEF1%) predito de 80,8±22,6 e mediana do percentual de IMC de 51,2 (1,3-97,7). No modelo de regressão múltipla, albumina, idade e percentual de IMC apresentaram associação com a função pulmonar. Indivíduos com IMC abaixo de 25% apresentaram VEF1% predito 12,2% menor. Um aumento de 0,1 mg de albumina teve associação com aumento de 2,7% no VEF1% predito, e um ano a mais de idade mostrou relação com a redução de 1,2% de VEF1% predito. Conclusão: O percentual de IMC, albumina e idade apresentaram associação independente com o VEF1% predito em um hospital terciário de referência.

OBJETIVO: Ânion gap corrigido e íon gap forte são usados comumente para estimar os ânions não medidos. Avaliamos o desempenho do ânion gap corrigido para albumina, fosfato e lactato na predição do íon gap forte em uma população mista de pacientes enfermos graves. Formulamos a hipótese de que o ânion gap corrigido para albumina, fosfato e lactato seria um bom preditor do íon gap forte, independentemente da presença de acidose metabólica. Além disso, avaliamos o impacto do íon gap forte por ocasião da admissão na mortalidade hospitalar. MÉTODOS: Incluímos 84 pacientes gravemente enfermos. A correlação e a concordância entre o ânion gap corrigido para albumina, fosfato e lactato e o íon gap forte foi avaliada utilizando-se os testes de correlação de Pearson, regressão linear, plot de Bland-Altman e pelo cálculo do coeficiente de correlação interclasse. Foram realizadas duas análises de subgrupos: uma para pacientes com excesso de base <-2mEq/L (grupo com baixo excesso de base) e outro grupo de pacientes com excesso de base >-2mEq/L (grupo com alto excesso de base). Foi realizada uma regressão logística para avaliar a associação entre os níveis de íon gap forte na admissão e a mortalidade hospitalar. RESULTADOS: Houve correlação muito forte e uma boa concordância entre o ânion gap corrigido para albumina, fosfato e lactato e o íon gap forte na população geral (r2=0,94; bias 1,40; limites de concordância de -0,75 a 3,57). A correlação foi também elevada nos grupos com baixo excesso de base (r2=0,94) e alto excesso de base (r2=0,92). OBJECTIVE: Corrected anion gap and strong ion gap are commonly used to estimate unmeasured anions. We evaluated the performance of the anion gap corrected for albumin, phosphate and lactate in predicting strong ion gap in a mixed population of critically ill patients. We hypothesized that anion gap corrected for albumin, phosphate and lactate would be a good predictor of strong ion gap, independent of the presence of metabolic acidosis. In addition, we evaluated the impact of strong ion gap at admission on hospital mortality. METHODS: We included 84 critically ill patients. Correlation and agreement between the anion gap corrected for albumin, phosphate and lactate and strong ion gap was evaluated by the Pearson correlation test, linear regression, a Bland-Altman plot and calculating interclass correlation coefficient. Two subgroup analyses were performed: one in patients with base-excess <-2mEq/L (low BE group - lBE) and the other in patients with base-excess >-2mEq/L (high BE group - hBE). A logistic regression was performed to evaluate the association between admission strong ion gap levels and hospital mortality. RESULTS: There was a very strong correlation and a good agreement between anion gap corrected for albumin, phosphate and lactate and strong ion gap in the general population (r2=0.94; bias 1.40; limits of agreement -0.75 to 3.57). Correlation was also high in the lBE group (r2=0.94) and in the hBE group (r2=0.92). High levels of strong ion gap were present in 66% of the whole population and 42% of the cases in the hBE group. Strong ion gap was not associated with hospital mortality by logistic regression. CONCLUSION: Anion gap corrected for albumin, phosphate and lactate and strong ion gap have an excellent correlation. Unmeasured anions are frequently elevated in critically ill patients with normal base-excess. However, there was no association between unmeasured anions and hospital mortality. .

OBJECTIVE: The objective of this study was to examine the effects of angiotensin-converting enzyme inhibitors on peritoneal membrane transport, peritoneal protein loss, and proteinuria in peritoneal dialysis patients. METHODS: Fifty-four peritoneal dialysis patients were included in the study. The patients were divided into two groups. Group 1 (n = 34) was treated with angiotensin-converting enzyme inhibitors. Group 2 (n = 20) did not receive any antihypertensive drugs during the entire follow-up. Eleven patients were excluded from the study thereafter. Thus, a total of 30 patients in Group 1 and 13 patients in Group 2 completed the study. We observed the patients for six months. Group 1 patients received maximal doses of angiotensin-converting enzyme inhibitors for six months. Parameters at the beginning of study and at the end of six months were evaluated. RESULTS: At the end of six months, total peritoneal protein loss in 24-hour dialysate effluent was significantly decreased in Group 1, whereas it was increased in Group 2. Compared to the baseline level, peritoneal albumin loss in 24-hour dialysate effluent and 4-hour D/P creatinine were significantly increased in Group 2 but were not significantly changed in Group 1. A covariance analysis between the groups revealed a significant difference only in the decreased amount of total protein loss in 24-hour dialysate. Proteinuria was decreased significantly in Group 1. CONCLUSION: This study suggests that angiotensin-converting enzyme inhibitors reduce peritoneal protein loss and small-solute transport and effectively protect peritoneal membrane transport in peritoneal dialysis patients.

ABSTRACT

Context : To evaluate the usefulness of urinary albumin excretion rate (UAER) i.e. Albumin/Creatinine Ratio (ACR) in diagnosis and prognosis of essential hypertension (EHT). Objectives : To find out the association of urinary albumin excretion rate with the pathophysiology of essential hypertension. Study Design : A cross-sectional analytical study. Materials & Methods : Urinary albumin excretion (UAE), urinary creatinine (UC) and UAER were analyzed and compared between hypertensive cases and age & sex matched normotensive controls of age group 30-65 years using unpaired two-tailed Student ‘t’ test. All statistical analyses were done with PASW (SPSS) v.18.0. Results : Systolic BP (SBP) and diastolic BP (DBP) of cases were found to be significantly higher (p < 0.001) than controls. Urine MAlb level (p < 0.001) and ACR (p < 0.001) in cases were significantly higher compared to controls. Correlation studies showed that SBP and DBP was significantly positively correlated with urine MAlb (SBP: r = 0.859, DBP: r = 0.733; p < 0.001) and ACR (SBP: r = 0.830, DBP: r = 0.739; p < 0.001). Sex-wise comparison in cases revealed that males had statistically non-significant (p > 0.05) lower levels of urine MAlb as compared to females but had significantly higher (p < 0.001) levels of urine creatinine and lower (p < 0.001) ACR compared to females. Conclusion: Urinary MAlb levels and ACR are seen to be increased in hypertensive subjects compared to normotensive subjects. ACR was significantly higher in female hypertensives than males which can be credited to the physiologically observed lower urine creatinine levels compared to males. Both Microalbuminuria and ACR can serve as specific and well-established marker of cardiovascular and renal damage in EHT.

ABSTRACT

Diabetic nephropathy (DN) is a progressive kidney disease that is caused by injury to kidney glomeruli. Podocytes are glomerular epithelial cells and play critical roles in the glomerular filtration barrier. Recent studies have shown the importance of regulating the podocyte actin cytoskeleton in early DN. The phosphoinositide 3-kinase (PI3K) inhibitor, wortmannin, simultaneously regulates Rac1 and Cdc42, which destabilize the podocyte actin cytoskeleton during early DN. In this study, in order to evaluate the reno-protective effects of wortmannin in early DN by regulating Rac1 and Cdc42, streptozotocin (STZ)-induced proteinuric renal disease (SPRD) rats were treated with wortmannin. The albuminuria value of the SPRD group was 3.55 +/- 0.56 mg/day, whereas wortmannin group was 1.77 +/- 0.48 mg/day. Also, the albumin to creatinine ratio (ACR) value of the SPRD group was 53.08 +/- 10.82 mg/g, whereas wortmannin group was 20.27 +/- 6.41 mg/g. Changes in the expression level of nephrin, podocin and Rac1/Cdc42, which is related to actin cytoskeleton in podocytes, by wortmannin administration were confirmed by Western blotting. The expression levels of nephrin (79.66 +/- 0.02), podocin (87.81 +/- 0.03) and Rac1/Cdc42 (86.12 +/- 0.02) in the wortmannin group were higher than the expression levels of nephrin (55.32 +/- 0.03), podocin (53.40 +/- 0.06) and Rac1/Cdc42 (54.05 +/- 0.04) in the SPRD group. In addition, expression and localization of nephrin, podocin and desmin were confirmed by immunofluorescence. In summary, we found for the first time that wortmannin has a reno-protective effect on SPRD rats during the early DN. The beneficial effects of wortmannin in SPRD rats indicate that this compound could be used to delay the progression of the disease during the early DN stage.

FUNDAMENTO: Considerou-se o uso indiscriminado de esteroides tanto por atletas de elite quanto por praticantes de atividades físicas. OBJETIVO: Avaliar os efeitos do decanoato de nandrolona sobre o perfil eletrocardiográfico, conteúdo glicogênico e de proteínas totais dos músculos cardíacos e esqueléticos, bem como as concentrações plasmática de albumina. MÉTODOS: Os animais do grupo tratado receberam a droga na concentração 5 mg/kg pela via subcutânea, duas vezes por semana, durante três semanas. Uma vez por semana, os ratos foram anestesiados com Pentobarbital sódico (50 mg/kg, ip) e submetidos à avaliação por meio do eletrocardiograma (ECG). Após o período experimental, amostras dos músculos cardíaco (ventrículo esquerdo - VE), sóleo (S), gastrocnêmio branco (GB), gastrocnêmio vermelho (GV), peitoral (P), intercostal (IC) e diafragma (D) foram prontamente coletadas e analisadas. Os dados (média ± epm) foram avaliados de acordo com ANOVA, segundo teste de Tukey (p>0,05). RESULTADOS: Os ratos do grupo tratado apresentaram alterações nos seguintes parâmetros cardíacos: intervalo QRS, intervalo QTc e frequência cardíaca, caracterizados por um aumento desses, tendo o ápice no intervalo da semana de pré-tratamento para a primeira semana. As reservas de glicogênio no VE apresentaram aumento de 127 por cento. Em relação à quantidade de proteínas totais, a diferença significativa foi constatada no S, GV e D. Quanto ao perfil bioquímico e ao hematócrito, foi observado um aumento na porcentagem de eritrócitos. CONCLUSÃO: O estudo mostra que importantes alterações cardíacas são deflagradas precocemente, sugerindo uma hierarquia na sequência de modificações que comprometem a homeostasia do organismo.


BACKGROUND: We considered both the indiscriminate use of steroids by top athletes and by physically active individuals. OBJECTIVE: To evaluate the effects of nandrolone decanoate on the electrocardiographic profile, glycogen content and total-protein profile of skeletal and cardiac muscles, as well as the plasma albumin concentrations. METHODS: The drug was administered subcutaneously, at a concentration of 5 mg/kg, twice a week for three weeks, to animals in the treated group. Once a week, the rats were anesthetized with sodium pentobarbital (50 mg/kg, ip) and they underwent an electrocardiogram (ECG). After the trial period, samples of the cardiac muscle (left ventricle - LV), soleus muscle (S), white gastrocnemius muscle (WG), red gastrocnemius muscle (RG), pectoral muscle (P), intercostal muscle (IC) and diaphragm muscle (D) were promptly collected and analyzed. An analysis of variance (ANOVA) and then a Tukey test (p>0.05) were carried out to assess the data (mean ± sem). RESULTS: There were changes in the following parameters of rats in the treated group: QRS interval, QTc interval and heart rate, characterized by an increase in these parameters, with the peak being reached in the period between the pre-treatment week and the first week. There was an increase of 127 percent in glycogen reserves in the LV. In relation to the total-protein amount, the significant difference was found in S, RG and D. As for the hematocrit and biochemical profile, it was possible to notice an increase in the percentage of erythrocytes. CONCLUSION: The study shows that major cardiac changes are triggered at an early stage, which indicates a hierarchy in the sequence of changes that compromise the homeostasis of the body.

OBJETIVO: Evaluar los efectos del decanoato de nandrolona sobre el perfil electrocardiográfico, contenido glicogénico y de proteínas totales de los músculos cardíacos y esqueléticos, así como las concentraciones plasmática de albúmina. MÉTODOS: Los animales del grupo tratado recibieron la droga en la concentración 5mg/kg por vía subcutánea, dos veces por semana, durante tres semanas. Una vez por semana, los ratones fueron anestesiados con Pentobarbital sódico (50mg/Kg, ip) y sometidos a evaluación por medio de electrocardiograma (ECG). Después del período experimental, muestras de los músculos cardíaco (ventrículo izquierdo - VI), sóleo (S), gastrocnemio blanco (GB), gastrocnemio rojo (GV), pectoral (P), intercostal (IC) y diafragma (D) fueron colectadas y analizadas. Los datos (media±epm) fueron evaluados de acuerdo con ANOVA, segundo test de Tukey (p>0,05). RESULTADOS: Los ratones del grupo tratado presentaron alteraciones en los siguientes parámetros cardíacos: intervalo QRS, intervalo QTc y frecuencia cardíaca, caracterizados por un aumento de estos, teniendo el ápice en el intervalo de la semana de pretratamiento a la primera semana. Las reservas de glicógeno en el VI presentaron aumento de 127 por ciento. En relación a la cantidad de proteínas totales, una diferencia significativa fue constatada en el S, GV y D. En cuanto al perfil bioquímico y al hematocrito, fue observado un aumento en el porcentaje de eritrocitos. CONCLUSIÓN: El estudio muestra que importantes alteraciones cardíacas son provocadas precozmente, sugiriendo una jerarquía en la secuencia de modificaciones que comprometen la homeostasia del organismo.

ABSTRACT

Background & objective: The efficacy of the combination of angiotensin receptor blockers (ARBs) and angiotensin converting enzyme (ACE) inhibitors in patients of type 1 diabetes mellitus (DM) with nephropathy is debatable. The antialbuminuric efficacy of dual blockade in patients of type 1 DM with micro- or macroabuminuria were evaluated. Methods: In this open label observational study 30 patients (20 male 10 female) with type 1 DM were included who were initially treated with telmisartan 80 mg for eight weeks followed by addition of ramipril 10 mg for a further eight weeks. Albuminuria reduction was studied at the end of each phase. Results: Therapy with telmisartan for 8 wk resulted in a 39 per cent (P<0.01) reduction in albumin excretion rate (AER). Combination therapy with telmisartan and ramipril produced a further reduction in AER of 33.4 per cent (P<0.01), amounting to a total AER reduction of 59 per cent (P<0.001). Dual blockade was more effective in the group of macroalbuminuric as compared to microalbuminuric subjects (P<0.05). Telmisartan produced a significant reduction in SBP (P<0.05). The addition of ramipril produced a further reduction in BP, the total reduction being 10.3 in SBP and 7.2 mmHg in DBP (P<0.001 for both). There was an increase in mean serum potassium of 0.39 mmol/l (P<0.01) from baseline at the end of the study period and two patients had hyperkalemia > 5.5 mmol/l with dual blockade. Interpretation & conclusion: Dual blockade with ramipril enhanced the antialbuminuric efficacy of telmisartan and further reduced blood pressure. The effect of dual blockade was more pronounced in the macroalbuminuric subjects and it was well tolerated. However, careful monitoring of serum potassium is required.

ABSTRACT

The clinical symptoms of diabetes mellitus in oral cavity are xerostomia, pathological changes in periodontal tissues and caries. It has been suggested that the determination of salvia's components in diabetic patients may be useful in describing and further understanding the oral findings in this condition. The aim of this study was to detect differences between the salivary albumin in diabetic patients and healthy people. A case-control study was carried out using data from Hamadan researches diabetes center in 2008. We selected 40 patients with type 1 diabetes mellitus aged 9-61 years, 40 patients with type2 aged 39-82 years and 40 healthy controls aged 20-67 years. Dental and oral status was assessed for all subjects. Completely unstimulated saliva samples were collected from diabetic patients and control subjects. Nephelometric method was used to determine salivary albumin concentration. Statistical analysis was done by SPSS. No significant difference in salivary albumin concentration was found between the control group and type 1 diabetic patients but salivary albumin was significantly higher in type 2 diabetic patients in compare with control groups [P=0.017], also no significant difference was found between type 1 and type 2 diabetic patients. In diabetic patients, complain of dry mouth and burning feeling in mouth was significantly higher, as well as, dental examinations showed higher DMFT mean values than the controls. Adults with type 2 diabetes mellitus have higher concentration of salivary albumin that would play a helpful diagnostic role in oral findings of diabetic patients

The aims of our study were to identify the risk factors for an increased aortic pulse wave velocity (AoPWV) and to assess the impact of the AoPWV on the cerebro-cardiovascular (CV) outcomes of hemodialysis (HD) patients. Seventy two HD patients were included, and the AoPWV, the echocardiography and the biochemical parameters were measured. After dividing the patients into tertiles according to the AoPWV values, we defined the low, the middle and the high AoPWV groups. The patients in the high AoPWV group showed a significantly higher age and high-sensitivity C-reactive protein level, a greater prevalence of diabetes and statin use, left ventricular hypertrophy, average pulse pressure (PP), AoPWV and left ventricular mass index and a lower serum albumin level than those in the low AoPWV group (p<0.05). On multivariate regression analysis of the AoPWV, age and the average PP were independently related to the AoPWV (p<0.05). On the multivariate Cox analysis for CV outcomes, the AoPWV and the average PP remained significant independent predictors of CV events. Our data suggest that an increased AoPWV is an independent predictor for the CV outcomes of HD patients.

ABSTRACT

AIM: to have the picture of infectious disease pattern and serum albumin level in elderly people with respiratory tract infection hospitalized at the Internal Medicine Ward, Dr. Moewardi Hospital Surakarta. METHODS: descriptive-retrospective study. Sample was taken from the medical records of patients hospitalized at the Internal Medicine Ward DR. Moewardi Hospital from January to December 2004. Infectious disease includes the ones at the respiratory tract (pulmonary tuberculosis (TB), pneumonia, chronic obstructive pulmonary disease (COPD)), urinary tract infection (UTI), digestive tract infection (gastroenteritis, typhoid fever), and tetanus. Serum albumin level data of patients with respiratory tract infection were taken from the hospital's laboratory results. RESULTS: from 337 elderly patients hospitalized at the Internal Medicine Ward DR. Moewardi Hospital Surakarta, proportion of patients aged 70 years or above was 39.17%, aged 60-64 years old were 37.09%, and 66-69 years were 23.74%. Distribution of diseases according to frequency is respiratory tract infection (66.77%), urinary tract infection (15.73 %), digestive tract infection (15.43%) (12.17%) and tetanus (2.07 %). From 57 elderly with respiratory tract infection, we found 45 individuals (78.95%) with hypoalbuminemia. CONCLUSION: the most common infectious disease found in the elderly hospitalized at Dr. Moewardi Hospital Surakarta is respiratory tract infection. Hypoalbuminemia was found in 78.95% of 57 elderly with respiratory tract infection.

ABSTRACT

OBJETIVO: Analisar a relação entre obesidade e taxa de excreção urinária de albumina (TEUA). MÉTODOS: Este é um estudo de corte de 73 pacientes com diabetes tipo 2 (DM) normoalbuminúricos (TEUA < 20 µg/min), que foram divididos em dois grupos de acordo com a mediana da TEUA (7,5 µg/min). A pressão sanguínea auscultatória foi medida em consultório com o paciente sentado, após repouso de 10min. RESULTADOS: A relação cintura:quadril (RCQ) foi maior no grupo com TEUA > 7,0 µg/min (0,98 ± 0,08), em comparação com o outro grupo (0,93 ± 0,08, p= 0,01). A RCQ correlacionou-se significantemente com a TEUA (r= 0,28; p= 0,018). Na análise de regressão linear múltipla, somente a RCQ associou-se com a TEUA (r= 0,29; p= 0,04). A HbA1c, a duração do DM e os níveis da PA sistólica foram excluídos do modelo.CONCLUSÕES: Pacientes com DM2 normoalbuminúricos com níveis mais elevados de TEUA podem representar um grupo com risco mais elevado para o desenvolvimento de doenças cardiovasculares.

ABSTRACT

Los hepatocitos son células epiteliales polarizadas que, al ser aisladas y cultivadas, pierden la polaridad y las propiedades de célula diferenciada. El cultivo de células hepáticas como esferoides permite obtener estructuras con organización de tipo tisular. En este trabajo se analizó estructural y funcionalmente la polaridad de esferoides porcinos. Para ello, las células hepáticas porcinas fueron aisladas y cultivadas en agitación constante. La actividad metabólica de los esferoides fue probada mediante el metabolismo de diazepam y de amonio, así como con síntesis de albúmina. Sus características estructurales mostraron la polaridad de las células. Fueron observados paquetes de fibras de colágeno distribuidas irregularmente y fibras reticulares en formahomogénea en todo el volumen del esferoide. Se hallaron células Ck19+ formando estructuras tipo ducto biliar, así como también _ y _-cateninas y pan-cadherinas en diferentes zonas, especialmente en las laminas externas, con características de epitelio cuboidal. Por microscopía electrónica de barrido se observaron estructuras muy compactas con superficie lisa, y por microscopía electrónica de transmisión, canalículos biliares con microvellosidades, uniones tight, zonula adherens y desmosomas. Las organelas celulares como mitocondrias, núcleos, nucleolos, peroxisomas, retículo endoplásmico estaban bien conservadas. Una compleja red de canalículos biliares fue observada mediante la incorporación y excreción de un análogo de sal biliar fluorescente. El análisis de los ácidos biliares excretados mostró un patrón normal. La morfología y funcionalidad de los esferoides puede aportar un modelo apropiado para aplicaciones en las que es primordial mantener las funciones específicas del hígado, como un dispositivo de hígado bioartificial.


Hepatocytes are epithelial cells that show a complex polarity in vivo. However, hepatocytes isolated and cultured in vitro normally lose both their differentiated properties and polarity. Culturing hepatocyte spheroids seems to be the accurate approach to maintain tissue level of organization. The structural and functional polaritiesof pig liver spheroids were analyzed in this work. Swine liver cells were isolated and cultured as spheroids. Their metabolic activity was proved through the metabolism of diazepam, ammonium and synthesis of albumin. Several structural features show the presence of polarity in the cells inside the spheroids. Reticular and collagen fibers, as well as Ck19(+) cells forming duct-like structures were found. _eta and _-catenins and pancadherins were positive in different regions of the spheroids, mainly in the outer cell layers, which have cuboidal epithelia features. The scanning electron microscopy showed a tightly compacted architecture, with smooth surface. The transmission electron microscopy analysis showed bile canaliculi with microvilli, tight junctions, zonula adherens and desmosome-like junctions. Wellmaintained cellular organelles, as mitochondria, nucleus,nucleolus, peroxisomes, endoplasmic reticulum, were seen in the spheroids. A complex inner bile canaliculinetwork was shown by using a fluorescent bile acid analogue incorporated and excreted by the spheroids. Furthermore, excretion of a normal pattern of bile acids was demonstrated. The morphology and functionality of the spheroids may provide an appropriate model for applications where the maintenance of liver-specific functions is crucial, as a bioartificial liver device.

ABSTRACT

The aim of this study was to identify the main factor affecting compliance and intima-media thickness of the elastic common carotid artery in continuous ambulatory peritoneal dialysis patients. Increased intima-media thickness and decreased arterial compliance are associated with elevated risk of cardiovascular disease. This study included 20 patients and 20 age- and sex- matched healthy control subjects. The compliance and intima-media thickness of the right common carotid artery within 1 cm to the bifurcation were measured three times using high-resolution B-mode echocardiography. Blood samples were obtained to measure levels of hemoglobin, phosphorus, total calcium, total CO2, serum albumin, C-reactive protein, serum total cholesterol, LDL- and HDL-cholesterol and triglycerides. We found that the compliance of common carotid artery was lower in the patient group than in the control group. In the patient group, the compliance of common carotid artery was positively correlated with serum albumin concentration, and intima-media thickness of common carotid artery was negatively correlated with serum albumin levels. Stepwise regression analysis showed that serum albumin concentration was independently related to the compliance of common carotid artery, suggesting hypoalbuminemia can independently give deleterious effects on the arterial wall in continuous ambulatory peritoneal dialysis patients.

ABSTRACT

This study was undertaken to clarify the effects of propofol on endotoxin-induced acute lung injury. Rabbits were randomly assigned to one of four groups. Each group received intravenous infusion of saline only, saline and Escherichia coli endotoxin, propofol (1 mg/kg bolus, then 5 mg/kg/hr) and endotoxin, or propofol (4 mg/kg bolus, then 20 mg/kg/hr) and endotoxin respectively. Infusion of saline or propofol was started 0.5 hr before the infusion of saline or endotoxin, and continued for 6 hr thereafter. The lungs of rabbits were ventilated with 40% oxygen. Mean blood pressure, heart rate, arterial oxygen tension (PaO2), and peripheral blood leukocyte and platelet count were recorded. The wet/dry (W/D) weight ratio of lung and lung injury score were measured, and analysis of bronchoalveolar lavage fluid (BALF) was done. Endotoxin decreased PaO2, and peripheral blood leukocyte and platelet count. And it increased W/D ratio of lung, lung injury score and leukocyte count, percentage of PMN cells, concentration of albumin, thromboxane B2 and IL-8 in BALF. Propofol attenuated all these changes except the leukocyte count in peripheral blood. In conclusion, propofol attenuated endotoxin-induced acute lung injury in rabbits mainly by inhibiting neutrophil and IL-8 responses, which may play a central role in sepsis-related lung injury.

ABSTRACT

Primary rat hepatocytes were cultured using different in vitro models and the enzyme leakage, albumin secretion, and cytochrome P450 1A (CYP 1A) activity were observed. The results showed that the level of LDH was decreased over time in culture. However, on day 5, LDH showed a significant increase in monolayer culture (MC) while after day 8 no LDH was detectable in sandwich culture (SC). The levels of AST and ALT did not change significantly over the investigated time. The CYP 1A activity was gradually decreased in a time-dependent manner in MC and SC. The decline of CYP 1A was faster in MC than in SC. This effect was partially reversed by using cytochrome P450 (CYP450) inducer such as Omeprazol and 3-methylcholanthrene (3-MC) and the CYP 1A induction was always higher in MC than in SC. In bioreactor basic CYP 1A activity was preserved over 2 weeks and the highest albumin production was observed in bioreactor followed by SC and MC. Taken together, it was indicated each investigated model had its advantages and disadvantages. It was also underlined that various in vitro models may address different questions.

ABSTRACT

To determine the influence of residual ß-cell function on retinopathy and microalbuminuria we measured basal C-peptide in 50 type 1 diabetic outpatients aged 24.96 + or - 7.14 years, with a duration of diabetes of 9.1 + or - 6.2 years. Forty-three patients (86 percent) with low C-peptide (<0.74 ng/ml) had longer duration of diabetes than 7 patients (14 percent) with high C-peptide (<0.74 ng/ml) (9 (2-34) vs 3 (1-10) years, P = 0.01) and a tendency to high glycated hemoglobin (HBA1) (8.8 (6-17.9) vs 7.7 (6.9-8.7)percent, P = 0.08). Nine patients (18 percent) had microalbuminuria (two out of three overnight urine samples with an albumin excretion rate (AER)> or - 20 and <200 µg/min) and 13 (26 percent) had background retinopathy. No association was found between low C-peptide, microalbuminuria and retinopathy and no difference in basal C-peptide was observed between microalbuminuric and normoalbuminuric patients (0.4 + or - 0.5 vs 0.19 + or - 0.22 ng/ml, P = 0.61) and between patients with or without retinopathy (0.4 + or - 0.6 vs 0.2 + or - 0.3 ng/ml, P = 0.43). Multiple regression analysis showed that duration of diabetes (r = 0.30, r2 = 0.09, P = 0.031) followed by HBA1 (r = 0.41, r2 = 0.17, P = 0.01) influenced basal C-peptide, and this duration of diabetes was the only variable affecting AER (r = 0.40, r2 = 0.16, P = 0.004). In our sample of type 1 diabetic patients residual ß-cell function was not associated with microalbuminuria or retinopathy

ABSTRACT

Se realizó un estudio prospectivo en 52 lesionados, y se estableció la correlación existente entre los valores de los índices predictivos utilizados y los resultados obtenidos en las fracciones protéicas estudiadas al ingreso, 3 y 7 días. Fueron aplicadas las siguientes pruebas estadísticas: medias, desviación estándar, minímo, máximo y prueba de correlación-regresión e hipótesis con una confiabilidad de 95 por ciento (alfa =0,05). El 38,5 por ciento de los lesionados correspondieron al rango entre 21 y 30 años de edad y predominó el sexo masculino (88,5 por ciento). Los accidentes del tránsito constituyeron la causa más frecuente de lesiones (38,5 por ciento); las lesiones ortopédicas fueron las de mayor incidencia (26,4 por ciento) y se encontraron complicaciones en el 38,4 por ciento de la muestra. Las proteínas totales, las albúminas y las globulinas gamma sufrieron disminución significativa a las 72 horas de ocurridas las lesiones, esta depresión se hizo más intensa en los fallecidos y se constató que existe correlación entre las variables estudiadas y los índices predictivos

ABSTRACT

Objetivo: describir la relación entre las concentraciones normales de IgAs y la albúmina salival en niños normales. Material y método: se midieron los concentraciones de IgAs, albúmina y razón 19AS/albúmina en saliva de 60 niños sanos de 0 a 9 años. Resultados: la concentración de IgAs no se distribuyó normalmente en la población estudiada y la distribución no se normalizó al corregir por albúmina. Los puntos de corte establecidos en base a los percentile, 2,5 y 97,5 en el conjunto de la muestra fueron 40,2 y 788 mg/L respectivamente, siendo el promedio de 239,03 m/L. No hubo diferencia estadísticamente significativa entre grupos por edad. Conclusiones: Aún no existe suficiente información sobre la utilidad clínica de la razón IgAs/albúmina, la cual debe evaluarse
